show profile settings take precedence over source profile settings.
Check the settings in the show profile per channel and consider changing them to "Retain Source settings"

I've seen this sort of behavior before when a switch isn't configured or aspects of the switch are not correctly configured. Make sure the switch is configured per the online documents.

If you are asking to assign an output port of the iQ to do two different sources (backfeed on left and PGM1 on right), this isn't possible.

To configure an iQ call controller with a VX engine: iQ has 4 show profile options. Each has a phone connection options. Edit a show profile. Looking up the release notes for that version on the Telos-Systems.com web site shows that version was released in 2007. I would suspect a hardware issue or some external trigger. I've not heard of any reports.
